 This is a full-time supervisory position in Sea Mar’s Care Management Program.  The position is based out of the Sea Mar Bellingham clinic.  The Care Manager supervises an interdisciplinary team who provides care management and care coordination services to high-risk patients with behavioral health and medical concerns in order to meet complex needs and to achieve better health outcomes. The team of nurses, social and health workers provide in-home or in-clinic assessments; assist the patient in the development of patient-centered health action plans, provide care coordination, and transition of care services.  The interdisciplinary care team will work toward better health outcomes by applying motivational interviewing and health coaching to empower patients to improve their self-management skills.

 

The Care Manager will support the team in maintaining an appropriate caseload of clients from various referral sources including health plans and internal referrals. The Care manager will collaborate with individuals within the Sea-Mar system and with community partners to promote integration of care and services and support whole person care for clients.  This is a dynamic position oriented toward health care transformation and fully integrated care. 

 

Qualifications:  Master’s degree in Social Work, Counseling, Public Health, Health Care Administration, or RN, preferred.  At least one year of supervisory experience required.  Will consider Bachelor degree with 3 years’ experience in care coordination or case management.  Spanish speaking preferred but not required.
