Temporary Full-time position for the Maternity Support Services Program in Mount Vernon. The duration for this temporary position will be four months. This position requires flexibility, initiative, and the ability to manage time, resources, and client caseload efficiently. Position requires the ability to work with many diverse individuals including medical providers, patients, care coordinators, the MSS team, and the community. Position may require travel to various sites within Clark County on a daily basis and very occasional travel to other areas within the state of Washington.  Must have access to an insured vehicle and be able to make home visits.

 

Responsibilities include providing outreach to and enrolling, recruiting and supporting MSS patients. Under the direction of an RN & Social Worker, may follow up on clients in their homes and deliver basic health messages. Ensure the utilization of medical care and other community resources are met. Responsible for administrative task including reception, phone calls, management of case files and E H R medical records.
