Full-time position for our Homeless Program in Mt. Vernon. We are seeking an energetic, self-directed individual who is responsible for ensuring individuals who are homeless (including migrants) have access to quality health care. This includes primary health care (including preventive health and dental care), substance abuse (drugs and alcohol) treatment and counseling, and mental health services. Individual will make all efforts to reduce barriers to care which includes case management services such as assistance with entitlement applications, translations, referrals for shelter, employment and job training, among others. We are looking for a highly motivated individual who is a self-starter, team player, dependable and has excellent customer service skills. Must be organized and with demonstrated ability to identify, initiate and coordinate special projects of various sizes. Individual must be at ease with homeless and migrant populations as position requires outreach at shelters and migrant camps, and have demonstrated proficiency with Microsoft applications (specifically Word, Excel and PowerPoint). Knowledge of electronic medical records desired but not required. Individual must be flexible with work schedule in order to best meet client needs (periodic weekends and/or weeknights required). Bilingual in English and Spanish is required; ability to speak Mixteco or Aguacateco preferred.

The Migrant Outreach Worker is a temporary part-time position to provide support and outreach to our Migrant and Seasonal Farmworker communities. This job description does not suggest or imply that these are the only duties to be performed by this employee. The Migrant Outreach Worker will be required to follow other instructions and to perform other duties as assigned by their supervisor. Setting up and taking down mobile medical clinics at outreach sites –at migrant camps, camps and other gathering sites. Lifting and loading all equipment/supplies into a van then unloading supplies to set up clinics. Participate in health promotional games and activities with adults and/or children depending on language skills. Able to work one to two days per week for 8-11 hours each day between June 25-August 31 (total worked will be limited to 20 hours every two weeks).Each shift begins and ends at offices in Mount Vernon. Adjustable shifts based on days clinics are held and location – usually not before 12noon, and most end by 9:30pm-10pm.
