This is a full-time position in Seattle.  The successful applicant will devote approximately .5 FTE to program coordination, including staff supervision and .5 FTE to direct patient service for the Health Education Program.   The Health Education Program staff are members of the Sea-Mar Community Health Center Interdisciplinary Care Teams, working in a fully integrated manner with Providers, and other teams such as the Chronic Care, and Care Management teams; working both, to provide health education services in a population health based model and to individual patients as may best meet the needs and goals of the organizational service delivery model and the patients that we serve. 

 

Primary responsibilities include but are not limited to

 

  • staff recruitment, selection, hiring and training, program planning and development to include both, in-clinic and community based services, working as an integral part of the Medical and other departments to best meet patient needs;

  • Evaluate program efficacy relative to patient outcomes, patient experience, etc.

  • Work closely with Supervisor, content experts, etc. to ensure both patient and staff training is consistent with best practices and organizational focus.

  • The Health Education Coordinator participates in departmental and organizational quality improvement activities, and collaborates with the Communications Coordinator for community health promotion activities and events.

  • Assists with grant writing and other program sustainability activities.
