Chemical Dependency Professional or CDPT: Full-time position available for new Behavioral Health Program in Aberdeen. CDP must have an AA degree and DASA Certification and must have a Washington State CDP License. Bilingual in English/Spanish is preferred. Chemical Dependency Professional Trainee:  The CDPT is a person who is in training to become a Chemical Dependency Professional. He/She will learn all of the duties and responsibilities of a CDP. Care Coordinator: Full-time position available in Bellingham. This position coordinates the monitoring and follow-up of patients requiring ongoing management of their care with a focus on patients with chronic conditions, in order to improve patient outcomes and clinic flow. The Care Coordinator is responsible for being part of the clinical team and enhancing quality and patient-centered care at Sea Mar Community Health Centers. Connect patients to Sea Mar and non-Sea Mar resources as appropriate and track all resources available to patients.  Assist patients with ongoing self-management goal setting based on mutual goal setting and with emphasis on client decision-making. Track patient’s adherence with plan of care in electronic charts.

 

Education and Experience: LPN or Medical Assistant II or BA/BSW with 2+ years of experience working with patients preferably in a community health care setting or equivalent in years of experience in a clinical setting.  Bilingual in English/Spanish required.

The main function is to supervise the receptionists, coordinate patient flow and coordinate patient schedules. Knowledge of MISYS (the current program used) desirable.
