Sea Mar Community Health Center’s Health Education Program
Program Overview

Health Educators are part of the Preventive Health Services Department (PHS) and work closely with all departments at Sea Mar to ensure comprehensive health services are provided to Sea Mar patients.

The health education team takes into account the sociocultural background of the patient and works to reduce barriers to improved health. Our services are informed by the social determinants of health, which we feel are essential to understanding how to make a lasting difference in the health of our patients.

Health Educators:

• Support patients living with chronic diseases through one-on-one appointments, groups, and classes
• Use motivational interviewing and evidence based information to help patients understand their health goals and overcome barriers
• Cover chronic disease management (diabetes, pre diabetes, cholesterol, and blood pressure), glucometer teaching, basic nutrition, asthma, tobacco cessation, family planning, exercise, and goal setting
• Offer a variety of classes including exercise groups, cooking workshops, yoga, diabetes management, and gardening.

Please note that Health Educators offer basic nutrition information. We are not certified nutritionists and cannot provide specific meal plans or specialized nutrition information. Patients who require more specific expertise must be referred to a nutritionist. Registered Dietitians and Health Educators at your clinic will work together to connect patients to health management classes and appointments.
